Must be a resident of Anaheim or Santa Ana and age 65 or older
Proper Identification will be required
Present your Photo ID AND any of the following:
- Driver’s license or California ID
- Utility Bill
- Social Security check, statement or letter
- Bank statement etc.
Bring your insurance card to the clinic e.g. Medicare, Medical etc.
Setting an appointment is really a joke. There is NEVER an appointment available. Don’t you know about this problem?
I know at least 5 people that made appointments and when I dropped them off at the location it was like a Disneyland line. Ros po , you just want someone to make the appointment for you huh.
